Major Critical Illness Benefit

  • First diagnosis, occurrence, or symptoms leading to the diagnosis happens after the 90-day no-claim period
  • You have survived the illness 14 days after the diagnosis

Minor Critical Illness Benefit

Get 20% of the benefit (capped at ₱500,000) amount if you are diagnosed with any of the 15 minor critical illnesses and if you meet the following conditions:

  • First diagnosis, occurrence, or symptoms leading to the diagnosis happens after the 90-day no-claim period
  • You have survived the illness 14 days after the diagnosis
  • If the minor critical illness is first diagnosed at least 1 year after you’ve been diagnosed with a major critical illness that received a benefit payout

Death Benefit

Healthy Life Benefit

How to File a Claim with FWD Life Insurance

1. Review your policy before filing a claim

Make sure your illness and/or situation is covered by your policy to avoid going through the trouble of gathering the requirements, only to find out that your insurance doesn’t cover it. If you’re not sure. The process is also applicable for loss of life claims or if you’re processing the claims for a loved one who needs assistance.

2. Prepare and send the requirements

The main requirements for Critical Illness and Hospitalization claims include:

  • Completed Claimant statement form of the insured or the beneficiaries, wherever applicable
  • Completely Attending physician's form
  • Certified True Copy of all the insured’s related medical/hospital records from recent and previous confinements and consultations. Examples are  Admitting History Records, medical certificates, Discharge Summaries, Clinical Abstracts, lab results, and all diagnostic results.
  • A copy of the Insured’s government ID with date of birth, photo, and signature
  • Any of the following as proof of a bank account 
    • Screenshot of the insured’s online bank account
    • ATM card
    • Statement of account
    • Validated deposit slip
    • Passbook

For Loss of Life claims, here are the main requirements:

  • The insured’s original copy of the Certified True Copy of the Death Certificate issued by the Local Civil Registrar (LCR) or the original copy issued by the Philippines Statistics Authority (PSA)
  • Completed Claimant’s Statement forms of the beneficiaries
  • Proof of relationship:
    • For spouse beneficiary: Original copy of Marriage Certificate issued by the PSA
    • For child beneficiary: Original copy of Birth Certificate issued by the PSA
    • For daughter with Single status in the contract and Married at the time of claim: Original copy of Marriage Certificate issued by the PSA or both copies of IDs with Single status and Married status
  • A copy of the Insured’s government ID with date of birth, photo, and signature
  • A copy of the beneficiary/ies government ID with date of birth, photo, and signature.
  • Any of the following as proof of a bank account with the account number and full name of beneficiary/ies visible:
    • Statement of account
    • ATM card
    • Validated deposit slip
    • Passbook
    • Screenshot of your online bank account

Below are the payment terms for the following insurance plans:

  • The One for Life - payment term is good for 1 year. If you continuously renew your The One for Life Insurance before it expires, you’ll have no premium increase after every 5 years.
  • Set for Life - 5, 7 or 10-year payment term, or whole life
  • Set for Health - 5 or 10-year payment term

Your coverage will start on the effective date written on the policy data page in your contract. Termination of insurance coverage depends on the type of insurance you have:

  • The One for Life- at the end of your payment term
  • Set for Life - coverage up to 100 years old
  • Set for Health - coverage until 75 years old

You can buy multiple plans for The One for Life Insurance as long as the maximum benefit amount doesn’t exceed ₱2 million.
